- Banner Medicare Advantage Prime HMO – Banner Medicare Advantage Prime HMO is a Medicare Advantage Part C Health Maintenance Organization. Banner Prime includes all the services under Original Medicare Parts A and B, as well as Medicare Part D prescription drug coverage and more. - Banner Medicare Advantage Dual HMO D-SNP – Banner Medicare Advantage Dual HMO D-SNP is specifically designed for people who are entitled to both Medicare and medical assistance from a state plan under Medicaid. In Arizona, the state Medicaid plan is called the Arizona Health Care Cost Containment System (AHCCCS). Banner Dual coordinates members’ Medicare and Medicaid (AHCCCS) benefits in one single plan and includes additional benefits like dental, hearing, vision, fitness, over-the-counter (OTC) allowances, routine transportation and more. Banner - University Family Care
Banner - University Family Care offers two types of health plans for participants in the Arizona Health Care Cost Containment System (AHCCCS), which is Arizona’s Medicaid program. To qualify for either plan, Arizona residents must meet certain income and eligibility requirements under AHCCCS.
- Banner - University Family Care AHCCCS Complete Care (B - UFC/ACC) is an integrated health care system that combines both physical and behavioral health services to improve coordination of care between providers and improve health care outcomes for members.
