Banner Medicare Advantage

Banner Medicare Advantage offers comprehensive benefits, prescription drug coverage and a large network of providers to choose from – making it easier for you to stay healthy. Find a Banner Medicare Advantage plan that works for you.

• Banner Medicare Advantage Prime HMO – Banner Medicare Advantage Prime HMO is a Medicare Advantage Part C Health Maintenance Organization. Banner Prime includes all the services under Original Medicare Parts A and B, as well as Medicare Part D prescription drug coverage and more. • Banner Medicare Advantage Dual HMO D-SNP – Banner Medicare Advantage Dual HMO D-SNP is specifically designed for people who are entitled to both Medicare and medical assistance from a state plan under Medicaid. In Arizona, the state Medicaid plan is called the Arizona Health Care Cost Containment System (AHCCCS). Banner Dual coordinates members’ Medicare and Medicaid (AHCCCS) benefits in one single plan and includes additional benefits like dental, hearing, vision, fitness, over-the-counter (OTC) allowances, routine transportation and more. Banner - University Family Care

Banner - University Family Care offers two types of health plans for participants in the Arizona Health Care Cost Containment System (AHCCCS), which is Arizona’s Medicaid program. To qualify for either plan, Arizona residents must meet certain income and eligibility requirements under AHCCCS.

• Banner - University Family Care AHCCCS Complete Care (B - UFC/ACC) is an integrated health care system that combines both physical and behavioral health services to improve coordination of care between providers and improve health care outcomes for members. • Banner - University Family Care Arizona Long Term Care System (B - UFC/ALTCS) is a specialized health plan designed for individuals aged 65 and older, as well as those with disabilities who require nursing facility-level care. This comprehensive plan delivers care and services across multiple settings, including assisted living facilities, members’ home, and various community-based environments. Banner Health Network

Banner Health Network is an integrated network for Medicare and private health plans. It is comprised of 3,000 Banner Health-affiliated physicians and advanced practice providers, 15 Phoenix-area Banner Health hospitals and a variety of other medical facilities.

Banner Network Colorado

Banner Network Colorado (BNC) is a provider network serving Medicare ACO and commercial policy holders in Larimer and Weld counties in northern Colorado’s Front Range. The comprehensive network includes more than 300 primary care physicians and specialists, three acute care hospitals, coupled with surgery centers, laboratory services, home care and medical equipment.

Banner Network Southern Arizona is a collaborative, clinically integrated network to serve Tucson and surrounding communities. The network features local physician leadership, coordinated care and technology tools that make patient care easier.
