Banner Health keeps a private, secure electronic medical record about your health. We offer our patients several options to access their health information.
Please note that a fee may be assessed for the cost of labor, supplies and/or postage in producing your record; you will be notified if any fees will be incurred. To reduce copy charges, you may request only your recent information or a specific document.
1. Enroll in your Patient Account, our patient portal, which provides access to your health information summary.
2. Obtain a copy of your medical records without the need to print, sign, and scan a copy of a request form. If you are a patient or the patient’s legal representative, click the appropriate link below to start the medical records request process with MRO Patient Central, a partner of Banner Health. Through this process, you can request your medical records from any Banner Health hospital, provider office, or urgent care location. Please have your government-issued photo ID available. If you are the parent of a minor child or the power of attorney for an adult patient, you will be asked to upload or provide additional documentation to verify that you are legally authorized to make this request.

3. Visit the Health Information Management Services (HIMS) department at any Banner Health hospital or the reception staff at your Banner Health provider’s office. Please have your government-issued photo ID available. If you are requesting records on behalf of a patient (i.e., personal representative, patient advocate, executor, trustee, or guardian), you will be asked to provide additional documentation to verify that you are legally authorized to make this request.
