At-Home Wound Care Program

If you have a wound that needs extra help to heal, you don’t have to do it alone. Our team of wound care specialists and home health nurses provides education, guidance and support right where you’re most comfortable - at home. 

At Banner Health, every home health nurse is trained in wound care, so you can count on skilled, compassionate care that helps you heal safely and confidently. 

What is at-home wound care? 

Sometimes wounds take longer to heal due to chronic conditions, circulation issues or a weakened immune system. Our at-home wound care program offers comprehensive care and education for you and your caregivers. 

We coordinate closely with your primary care provider and care team to design a healing plan that meets your unique needs. 

Common wounds we treat include: 

  • Pressure ulcers 
  • Diabetic ulcers 
  • Ostomy care 
  • Surgical wounds 
  • Wounds caused by infection 
  • Traumatic wounds 
  • Wounds related to circulatory or autoimmune conditions 

We provide intermittent in-home visits focused on education and wound management, helping you gain the skills and confidence to care for your wound between visits. 

For wounds that require advanced attention, Banner Health also offers wound care clinics for more specialized treatment.

Virtual home monitoring 

Our Virtual Home Monitoring program allows our team to track your healing between visits using secure digital tools. Through a tablet-based system, you and your care team can: 

  • Record and monitor daily vital signs 
  • Send updates and wound photos to your care team through a secure website 
  • Track medication and treatment progress 
  • Alert staff immediately for urgent concerns 

This daily connection helps ensure that your wounds are healing properly and that any issues are addressed quickly.

How often will my nurses visit? 

Every wound is different, so your visit schedule will depend on your condition and healing progress. Our nurses, your doctor and your caregivers will work together to create a care plan that balances in-person visits with virtual check-ins for the best outcomes. 

How we support self-care and education 

Our wound care team focuses on empowering you and your caregivers with the skills to manage wound care confidently. Education includes: 

  • Proper cleaning and dressing techniques 
  • Recognizing signs of infection 
  • Managing conditions that affect wound healing, such as diabetes or vascular disease 

How wound progress is tracked 

We use advanced technology, including Home Care Home Base, to document and monitor your progress from start to finish. This includes photos, wound labeling and detailed progress notes that help your care team make timely adjustments to your plan.
